Salary range: $40,000-42,000 per year

Job Overview:

As a Re-Grantee Community Organization Partnering with Baltimore CONNECT, we are seeking one full-time Community Health Worker (CHW) as part of the CEAL-DMV (Community Engagement Alliance Against COVID-19 Disparities) grant. This four-year grant aims to improve access to healthcare and social services for Baltimore City residents, with a focus on addressing key social determinants of health (SDOH) such as chronic disease management, housing, nutrition, and more.

CHWs will work under the supervision of the Baltimore CONNECT CHW Supervisor and partner with community-based organizations to empower individuals to take control of their health and well-being. This position will also involve the use of digital technology to identify resources and facilitate linkages for participants. The target population for this role includes Baltimore City residents aged 18 and older.

Key Responsibilities:

●        Conduct health coaching sessions with clients, focusing on topics such as nutrition, exercise, chronic disease prevention, and emotional wellness.

●        Educate community members on the importance of preventative care, wellness strategies, and healthcare access.

●        Assist clients in setting realistic health goals and developing personalized action plans to achieve them.

●        Facilitate group wellness workshops and support groups, both virtually and in person.

●        Provide resources and referrals to local healthcare services and community programs.

●        Document client interactions and progress in accordance with program guidelines.

●        Collaborate with healthcare providers, social service agencies, and other community organizations to coordinate care.

●        Conduct outreach activities to engage the community and promote services.

●        Maintain confidentiality and adhere to the highest ethical standards in client interactions.

●       Assist participants in using the Medi platform to access health and supportive services in keeping with the clients' needs.

●       As needed by clients, assist with navigation of and referral to services, with the aid of the Medi platform.

●       Accurately document encounters using Medi and other administrative tools made available by the CEAL DMV team.

Qualifications:

●      High school diploma or equivalent required;

●      Previous experience working as a Community Health Worker, health coach, or similar role. CHW certification required, or the ability to obtain CHW certification within 60 days of hire.

●      Experience facilitating group health education sessions or workshops.

●      Excellent communication and interpersonal skills with the ability to connect with diverse communities.

●      Knowledge of health and wellness practices, particularly related to chronic disease prevention, nutrition, and holistic health. Additional training will be provided.

●      Ability to work both independently and as part of a team.

●      Familiarity with Baltimore City communities and resources is a plus.

●      Must have reliable transportation

●      Proficiency in data collection and reporting using digital tools or electronic health record systems.

Additional Information:

●      Hours: 37.5 hours per week, Monday through Friday, with some weekend and evening hours as needed.

●      Duration: This is a grant-funded position renewed annually.

 

Reportability

●      Report regularly to CHW supervisor

●      Attend required meetings: supervision, team meetings, etc.

●      Provide weekly schedules to CHW supervisor

 

Accountability

●      Complete required trainings

●      Complete Agency requirements

●      Provide documentation accurately and timely
